2008 CONFERENCE REGISTRATION FORMHome

Fees include all conference materials, 3 breakfasts, 2 lunches, and breaks.
All attendees are responsible for their own travel and hotel arrangements.
Cancellations and requests for refunds must be made in writing and must be received by July 20, 2008.
No refunds will be made after July 20, 2008.
A block of rooms has been reserved at the
Sheraton Burlington Hotel (870 Williston Road, Burlington VT 05403).
Rate: $125 single/double occupancy. The deadline to receive the reduced rate is July 6, 2008.
